Property Maintenance & Complaint Form
This is a form for residents to file complaints about property maintenance violations or tenant-related issues in rental units. All information must be completed for the complaint to be processed.
What You Need to Provide
- Your name, address, phone number, and apartment location
- Address where the violation is occurring
- Detailed description of the issue(s)
- For tenant complaints: landlord/property manager contact information and timeline of your efforts to resolve the issue
Important Legal Notice
Submission of false statements to a public official pursuant to Section 4904 of Title 18 of the Pennsylvania Crimes Code constitutes a misdemeanor of the third degree, punishable by a fine and imprisonment of not more than one year.
You may be contacted to appear in court to testify about the violations noted in your complaint.
